Do not paste your entire membership agreement into the liability waiver field — Mindbody’s waiver screen is for risk acknowledgment. Use client forms for operational T&C.
Step 1: List the policies you need
Most boutique studios publish at least:

Step 2: Create a client form in Mindbody
- Open Mindbody Business Mode
- Navigate to Settings → Client Forms and Liability Waiver
- Choose Client Forms → New Form (or edit an existing form)
- Name the form clearly — e.g. “Membership Terms & Conditions 2026”
Mindbody’s support articles describe client forms as the place for custom questions, policies, and optional signatures beyond the base waiver.
Step 3: Add policy blocks and acknowledgements
For each T&C section:
- Add a Policy or Text block with your terms copy
- Enable Require client acknowledgement (checkbox) for each section members must agree to
- For high-stakes items (billing, auto-renew), enable Require client signature if your account supports it
- Order blocks logically: studio rules → billing → cancellation → liability cross-reference

If a member signed an old version, Mindbody will not retroactively apply a new signature requirement — resend the updated form.
