Step 1: Open Client Forms and Liability Waiver
- Sign in to Mindbody Business Mode
- Go to Settings (gear icon) or Manager Tools → Setup
- Open Client Forms and Liability Waiver (wording may appear as Client Forms on some accounts)
- Select Liability Waiver or Create / Edit Waiver

Step 2: Enter your waiver text
- Paste or type your liability waiver body copy
- Keep sections scannable — headers for assumption of risk, release, and medical disclosure

- Save the waiver

Step 3: Require acknowledgment at booking
To force new and returning clients to accept the waiver before booking:
- In Client Forms and Liability Waiver, enable Require waiver acknowledgement (or equivalent toggle)
- Choose whether acknowledgment applies to all clients or first visit only — policy varies by studio
- Save and test on the consumer booking path
Members booking on your Mindbody consumer site or app should see the waiver step before checkout completes.
Step 4: Enable e-signatures (optional)
Mindbody supports client signatures on forms and waivers when configured:
- Edit the waiver or linked client form
- Enable Require client signature on the policy block
- Resend the form to existing clients — they cannot retroactively sign an old checkbox-only version
Signature capture improves defensibility but adds friction. Many studios use signatures for membership contracts and checkboxes for class drop-ins.
Step 5: Test the member experience
